As a Hope PACE participant, you would receive all of your medical care through the program. Their prescriptions are included, along with careful medication management by the medical staff at Hope PACE. You can visit with medical specialists – even dentists. If you need assistance with personal care, light housekeeping or equipment and supplies, it\u2019s all available through PACE.<\/p>\n
\u201cMany of the social and medical services will take place at a Hope Care Centers such as the center in Lehigh Acres.\u201d said Dr. Sheeja Kanacheril\u00a0PACE Medical Director at\u00a0Hope HealthCare Services.<\/p>\n
While at a Center participants can enjoy nutritious, warm meals along with fun activities like Bingo, art therapy, games and contests. Musicians and pet visitors frequently drop by for your entertainment. You can enjoy light exercise with interactive Wii games, simple stretches and outdoor activities. Don\u2019t worry about how to get there \u2013 Hope can provide free transportation to and from their home.<\/p>\n
While the participant enjoying your day at the Care Center, and the participant caregiver can relax for a morning or afternoon of respite. Also the participant loved ones receive expert guidance and support so they can also improve their quality of life.<\/p>\n

Eligibility:<\/p>\n
To be eligible to enroll in Hope PACE you must:<\/p>\n
Be at least 55 years old<\/p>\n
Be a resident of Charlotte, Collier or Lee County<\/p>\n
Be able to live in the community without jeopardizing your health or safety<\/p>\n
Have health problems that qualify you for nursing home care<\/p>\n
Costs:<\/p>\n
Costs depend on your Medicare and Medicaid eligibility. For participants who are not Medicaid eligible, there is a fee. The program is also available to private pay participants.<\/p>\n
